DLI / Donor Lymphocyte Infusion

Bereits schon 1990 wurde durch Kolb et al. der erste Versuch unternommen, durch die Gabe von Spenderlymphozyten-Infusionen ("Donor Lymphocyte Infusion", DLI) einen GVL Effekt zu induzieren.
Als GVL-Effekt wird das Vorgehen von Spenderlymphozyten gegen im Empfängerorganismus verbliebene residuale Lymphomzellen bezeichnet.
Zur Wirksamkeit von DLI bei Rezidiven von Lymphomen nach allogener Stammzelltherapie gibt es zwar immer noch wenige Studien, diese weisen aber signifikante Erfolge auf.
Die DLI scheint bei einem Rückfall nach einer allogenen Stammzelltransplantation jedoch eine weitere Option für Patienten mit einem Lymphom zu sein. Allerdings tritt eine GvHD bei ca. 50-60 % der mittels DLI behandelten Patienten auf und kann mit anderen Komplikationen verbunden sein.
Die Entwicklung einer GvHD geht jedoch mit einem längeren rezidivfreien Überleben einher und ist mit einer nur geringen Mortalität assoziiert. Eine weitere Komplikation der DLI ist die Aplasie des Knochenmarks, welche bei 20-40 % der Patienten vorkommt.
